IESA Physical Form

Prior to tryouts all Hickory Creek Middle School athletes will need a current physical on file in the main office.

 

Please utilize the form below if able, although alternative forms will be accepted so long as they meet the criteria below.

 

The IESA requires all participating athletes to provide a certificate of physical fitness issued by a licensed physician, physician’s assistant, or nurse practitioner as set forth in the Illinois State Statutes not more than 395 days preceding such tryout, practice, or contest in any athletic activity.

 

Note: "Sports Physicals" last for 13 months however general physicals are good for 12 months from the date of the exam.
