Code of Conduct

All parents of Hickory Creek Athletes must sign-off that they fully understand and accept the policies and procedures for athletics at Hickory Creek. Please click on the link below and read through the policies and procedures we expect Hickory Creek athletes to follow throughout their season. You will only have to complete this once per school year for each child who participates.

Click the link below to fill out a form stating that you have completed the mandatory Code of Conduct and that you understand all HCMS Athletic policies and procedures.

 

This online form must be completed prior to participation in any athletic contests.
